About the Role
We're seeking a dynamic Care Experience Specialist to support clients and families throughout their treatment journey. You'll welcome new clients post-admission, build meaningful rapport, and serve as the primary point of contact for care coordination and customer service. Acting as a liaison between clients and internal teams, you'll ensure every person receives an exceptional experience and all their needs are met.
Key Responsibilities
- Answer inbound calls, emails, and texts, resolving patient and family concerns efficiently
- Act as liaison between patients and internal teams including Admissions, Billing, Utilization Review, Outreach, and Clinical
- Support client-related requests from the Clinical Care team to enhance patient experience
- Identify gaps in treatment attendance and proactively reach out to resolve barriers to engagement
- Communicate aftercare resources and schedule post-treatment appointments with families
- Manage client schedules and coordinate appointment scheduling and rescheduling
- Complete documentation accurately and timely
- Adapt to organizational changes to meet evolving client and team needs
